Abstract
Defects of the auricle may be secondary to a variety of etiologies. Most commonly it occurs from excision of skin cancers. Albeit small in size, they can be quite disfiguring due to the alteration in the auricles three-dimensional complex design. Postauricular skin with its wide vascular pedicle, thinness, and excellent color match provides an ideal donor site for advancement flaps to reconstruct defects of the middle third of the helix and/or antihelix. Additionally, this area is concealed behind the ear adding to its aesthetic value. This chapter thoroughly describes the two-step postauricular flap. Indications, preoperative markings, intraoperative steps, postoperative care, possible complications, and a template of an operative dictation are included.
