Abstract
Perforator flaps are the latest development in the evolution of tissue transfer. The use of perforator flaps in reconstruction of lower extremity wounds is reviewed. The history, development, advantages, disadvantages, and classification of perforator flaps are discussed. Specific perforator flaps are described, along with their use in reconstructing the lower extremity. Future directions of lower extremity reconstruction may involve flap thinning, compound perforator flaps, or supermicrosurgery.
